Chrissy accepts Chronic Disease Management Plans for Diabetes Education and/or Exercise Physiology. 

Your GP can provide an Enhanced Primary Care Plan (EPC) referral which will enable a Medicare rebate for these services.

*EPC referrals include five (5) appointments per year across all allied health practitioners.
