Chrissy accepts Chronic Condition Management Plans (CCMP) for Diabetes Education and/or Exercise Physiology. 

Your GP can provide an referral which will enable a Medicare rebate for these services.

*CCMP referrals include five (5) appointments per year across all allied health practitioners.
